Forms & Advocacy Assist Service

Need some help filling out a form or maybe some advice to get you started? The Forms and Advocacy Assist Service is here to help.

Prace in partnership with La Trobe University is offering this FREE service to the community.

The service can help with:

  • Centrelink – various forms
  • Housing and Lease agreements
  • Family violence/Personal Safety Intervention Order Applications
  • Rebates
  • Citizenship applications
  • Superannuation
  • Identity documents
  • Powers of Attorney and Medical Treatment Decision Maker forms
  • Fines
  • Statutory Declarations

The service also offers advocacy in a range of areas such as housing, complaints and job agencies.

Eligibility Criteria for FAAS – Merrilands Community Centre

To ensure that our service provides impactful, localised assistance, the following eligibility criteria applies.

To access the service, clients must either live, work or study in:

  • the City of Darebin, Whittlesea or Banyule, or
  • be connected to a Prace service.


Please note FAAS is not a legal service.

The weekly service is delivered by La Trobe University students by appointment on Tuesdays (for a limited time) and Thursdays at Merrilands Community Centre, Reservoir.
